F&SCM Security Form

Office of the Controller, FMIS System department has released a new Finance & Supply Chain Management (F&SCM) Security Form for requesting access to the F&SCM Enterprise Resource Planning (ERP) system.

Complete the form in its entirety and obtain all required signatures. Submit the form to the Systems office via email ooc.fmissupportteam@navajo-nsn.gov or deliver it to the Systems Department in the Administration Building 1.  

The F&SCM Security Form must be fully completed; incomplete submissions will not be processed. Once reviewed, access will be granted and you will receive an email confirmation containing instructions.
